To begin the membership process and reserve your first appointment, a $150 enrollment credit is required.

This is not an extra fee — it is fully applied toward your membership plan (Essential, Enhanced, or Signature).

This includes your initial consultation (30–60 minute visit).

After payment, patients will be prompted to schedule their appointment. At or before the visit, they will select a membership plan and we will securely add their card on file for recurring monthly billing.

Do I need insurance?

Yes. Modern Edge Family Practice accepts insurance for standard medical care visits. We currently accept Blue Cross Blue Shield of New Mexico, Presbyterian, UnitedHealthcare, and Aetna (Commercial & Medicare plans).

Patients without insurance may choose cash-pay options or explore one of our membership plans for enhanced wellness support and access.

What are the benefits of a membership?

Membership plans provide enhanced access, wellness coordination, longer appointments, priority scheduling, and additional support beyond traditional visits.

Enhanced and Signature memberships each include 2 complimentary standard visits annually.

Can I cancel my membership?

Yes. Memberships may be canceled after the 3-month commitment period with 30 days written notice.

Does insurance cover membership fees?

No. Membership fees are separate from insurance and cover enhanced services and wellness support. Insurance may still be used for eligible medical visits and services.
