Wegovy Pill, Foundayo, Tirzepatide, and Retatrutide: Which Weight-Loss Medications Are Available in 2026?

Weight-loss medicine changed quickly between late 2025 and 2026. The FDA approved the first two GLP-1 pills for chronic weight management, tirzepatide became the most searched weight-loss medication in the United States, and online interest in retatrutide rose by roughly two-thirds over the past year even though it is not approved. Patients in Albuquerque now ask us a simple question: which of these can I actually get, and which one fits me? Our medical weight loss program starts with that conversation.
Quick Answer
- Wegovy pill (oral semaglutide 25 mg): FDA-approved December 22, 2025. Once daily, taken on an empty stomach.
- Foundayo (orforglipron): FDA-approved April 1, 2026. Once-daily pill with no food or water timing rules.
- Tirzepatide (Zepbound, Mounjaro): FDA-approved weekly injection with the largest average weight loss among currently approved options.
- Retatrutide: Investigational. Not FDA-approved for any use, and not legally compoundable.
What Is the Wegovy Pill?
The Wegovy pill is oral semaglutide 25 mg, the same active ingredient as Wegovy injections in a once-daily tablet. It is approved for adults with obesity, or overweight with at least one weight-related condition, alongside diet and activity changes. It also carries a cardiovascular risk-reduction indication for adults with established heart disease.
In the 64-week OASIS 4 trial, average weight loss was 13.6% with the pill versus 2.4% with placebo across all participants, and 16.6% among people who stayed on treatment. Oral semaglutide must be taken first thing in the morning on an empty stomach with a small sip of water, then you wait before eating, drinking, or taking other oral medications. For patients who are consistent with a morning routine, that is easy. For others it is the main drawback.
What Is Foundayo (Orforglipron)?
Foundayo is a small-molecule GLP-1 receptor agonist from Eli Lilly, approved April 1, 2026 for chronic weight management. Unlike oral semaglutide, it does not require fasting or water restrictions, so it can be taken at any time of day. In its obesity trials, average weight loss was modestly lower than injectable tirzepatide or oral semaglutide, at roughly 12% at the highest dose, while improvements in cardiometabolic markers were broadly similar. A type 2 diabetes indication has not yet been approved.
Why Is Tirzepatide Still the Most Searched?
Google Trends data shows US search interest in tirzepatide rose about 30% over the past year, ahead of semaglutide. Tirzepatide activates both GIP and GLP-1 receptors. In the SURMOUNT-1 trial, people on the highest dose lost about 20.9% of body weight on average over 72 weeks. It remains a weekly injection, and for many patients it is still the most effective approved option. We cover dosing trade-offs and muscle preservation in Semaglutide vs. Tirzepatide and How to Prevent Muscle Loss.
What Is Retatrutide, and Can You Get It?
Retatrutide is an investigational once-weekly injection that targets three receptors: GIP, GLP-1, and glucagon. Lilly has reported average weight loss in the high-20% range in late-stage trials, which explains the search surge. However, as of October 2026 retatrutide is not FDA-approved, no approval date has been set, and the FDA has stated it cannot be used in pharmacy compounding.
Products sold online as "reta" or "research peptides" are not legitimate retatrutide prescriptions. Their identity, dose, sterility, and contamination cannot be verified. Lilly lists a pre-approval expanded access program on ClinicalTrials.gov, but that is a physician-requested pathway for specific patients, not a retail option.
Pill or Injection: How to Decide
| Option | Form | Approved | Average weight loss in trials | Key trade-off |
|---|---|---|---|---|
| Wegovy pill | Daily tablet | Yes (Dec 2025) | About 14% to 17% | Strict empty-stomach routine |
| Foundayo | Daily tablet | Yes (Apr 2026) | About 12% | Slightly less weight loss, no food rules |
| Tirzepatide | Weekly injection | Yes | About 21% at top dose | Injection, cost and supply |
| Retatrutide | Weekly injection | No | High 20s% (company-reported) | Not available outside trials |
The right choice depends on your weight goal, needle comfort, other medications (oral semaglutide can affect absorption of some drugs, such as levothyroxine), digestive side effects, insurance coverage, cost, and medical history. All GLP-1 medications share warnings about gastrointestinal effects, gallbladder disease, pancreatitis, and a contraindication for people with a personal or family history of medullary thyroid carcinoma or MEN2.
What Stays the Same No Matter Which You Choose
Every option works best with protein-forward nutrition, resistance training to protect muscle, and lab and body-composition monitoring. Weight regain is common after stopping any of these medications, which we explain in What Happens When You Stop Semaglutide or Tirzepatide. At Modern Edge, we pair medication decisions with comprehensive lab panels and InBody 580 scans so progress is measured as fat loss, not just pounds.
Frequently Asked Questions
Is the Wegovy pill as effective as the injection?
Results are similar. OASIS 4 showed 13.6% to 16.6% average weight loss with the 25 mg pill, comparable to the roughly 15% seen with weekly Wegovy injections in earlier trials.
Is Foundayo the same as the Wegovy pill?
No. Foundayo (orforglipron) is a different molecule made by Eli Lilly. It does not need to be taken on an empty stomach, while the Wegovy pill does.
When will retatrutide be approved?
No approval date exists as of October 2026. Any timeline online is a projection. Avoid products marketed as retatrutide outside of clinical trials.
Can I switch from an injection to a pill?
Sometimes. Switching requires a provider to plan the dose transition, because doses are not one-to-one between forms.
