What Happens When You Stop Semaglutide or Tirzepatide?

Appetite commonly increases and some weight regain is likely after stopping a GLP-1 based medication. That is physiology, not a failure of willpower.
Why Weight May Return
Semaglutide and tirzepatide influence appetite, fullness, food intake, and glucose regulation while they are being used. When treatment stops, those effects fade. Trials of semaglutide show substantial average regain after withdrawal, although individual outcomes vary.
Blood sugar, blood pressure, lipids, sleep apnea symptoms, or other weight-related conditions may also worsen as weight returns. People taking medication for diabetes need a specific transition plan to avoid loss of glucose control.
Do You Need to Taper?
These medicines do not cause classic drug dependence, and FDA labeling does not require tapering. A clinician may still use a step-down plan for comfort, cost, side-effect management, or close observation. Do not stretch doses, combine products, or improvise a taper without guidance.
Pregnancy planning requires special timing. Semaglutide labeling advises stopping at least two months before a planned pregnancy. Tirzepatide has different pharmacokinetics and should be discussed with the prescriber.
A Better Off-Ramp
Before stopping, identify the reason: side effects, cost, goal achievement, pregnancy, surgery, or another medical issue. Build a maintenance plan around protein and fiber intake, resistance training, sleep, weight trend, and treatment of binge eating or other drivers.
Some patients benefit from long-term medication, a lower maintenance dose, a different medication, or structured follow-up. The decision should reflect benefits, risks, access, and personal goals.
