Fasting Insulin vs. A1c: What Do They Tell You?

A1c and fasting insulin answer different questions. A1c is standardized for diagnosing and monitoring diabetes. Fasting insulin may add metabolic context but has important limitations.

What A1c Measures

A1c estimates average glucose exposure over roughly the prior two to three months, with greater influence from recent weeks. It does not require fasting and is widely standardized.

An A1c below 5.7% is generally considered normal, 5.7% to 6.4% is in the prediabetes range, and 6.5% or higher may indicate diabetes when confirmed appropriately. Pregnancy, anemia, recent blood loss or transfusion, kidney disease, and hemoglobin variants can make A1c misleading.

What Fasting Insulin Measures

Fasting insulin shows how much insulin is circulating after an overnight fast. A higher value may suggest the pancreas is compensating for insulin resistance before glucose becomes abnormal.

However, insulin assays are not standardized as well as glucose or A1c, reference ranges vary, and there is no universally accepted fasting-insulin cutoff for diagnosing insulin resistance. Stress, fasting duration, medications, and acute illness may affect the result.

Putting Results Together

Fasting glucose, A1c, triglycerides, HDL, waist circumference, blood pressure, clinical history, and sometimes an oral glucose tolerance test often provide the most actionable picture. Calculations such as HOMA-IR can be useful in research or selected clinical settings but should not be treated as a stand-alone diagnosis.
