Can Women Use Testosterone Therapy? Benefits, Risks, and Monitoring

Women naturally produce testosterone. Therapy can be appropriate in a narrow, evidence-based setting, but it should not be marketed as a universal cure for fatigue, weight gain, or aging.
Where the Evidence Is Strongest
The best-supported indication is hypoactive sexual desire disorder in carefully assessed postmenopausal women. HSDD is persistent low desire that causes distress and is not better explained by relationship factors, pain, medications, depression, thyroid disease, or another condition.
No testosterone product is FDA approved specifically for women in the United States. Clinicians who prescribe it generally use carefully reduced doses designed to keep blood levels in the normal premenopausal female range.
Possible Benefits and Limits
Some appropriately selected women experience improved sexual desire, arousal, orgasm, or sexual satisfaction. Evidence is not strong enough to routinely recommend testosterone for cognition, mood, energy, muscle gain, weight loss, or disease prevention.
Testosterone is also not a substitute for estrogen when hot flashes, night sweats, vaginal dryness, or bone protection are the primary issue.
Risks and Monitoring
Excess exposure may cause acne, unwanted facial or body hair, scalp hair thinning, oily skin, voice deepening, or clitoral enlargement. Some androgenic changes may not fully reverse. Long-term breast and cardiovascular safety data remain limited.
Monitoring includes symptoms, side effects, total testosterone, and sometimes SHBG. Levels should not be pushed above the physiologic female range. Pregnancy is a contraindication. Pellets and injections can make fine dose adjustment more difficult and may produce supraphysiologic levels.
