TRT Injections vs. Pellets vs. Gels: Comparing Your Options

All three methods can raise testosterone in appropriately diagnosed men. The practical differences come down to adjustability, convenience, cost, exposure risk, and how quickly treatment can be changed if something is not working.
Injections
Advantages include precise dose adjustment, predictable delivery, broad availability, and usually lower cost. Subcutaneous or intramuscular schedules can be individualized to the patient rather than forced into a single protocol.
Drawbacks include needles, supplies, adherence, and possible peaks and troughs between doses. Testosterone can increase hematocrit, worsen acne, contribute to hair loss in susceptible men, reduce fertility, and sometimes aggravate untreated severe sleep apnea.
Pellets
Pellets provide long intervals without daily or weekly dosing and may produce stable levels for some patients. They are attractive to people who travel frequently or prefer not to self-administer.
They do require a minor procedure. Dose changes are slow, and possible complications include bruising, infection, scarring, extrusion, or an undesirably high level that cannot be rapidly reversed.
Gels
Gels avoid needles and can provide steady daily exposure. Because they are adjusted or stopped easily, they are useful when flexibility matters.
Absorption varies from person to person. Daily application is required, and skin-to-skin transfer can expose partners or children. Washing hands, covering the application site, and following product-specific timing instructions are essential.
The Monitoring Is Similar Either Way
Regardless of delivery method, TRT should begin only after a proper diagnosis. Follow-up typically considers symptom response, testosterone level drawn at a formulation-appropriate time, hematocrit, adverse effects, fertility goals, and prostate assessment based on age and risk.
