Why Weight-Loss Plateaus Happen on GLP-1 Medication

A plateau does not mean semaglutide or tirzepatide has suddenly stopped working. Weight loss naturally slows as the body becomes smaller and energy needs fall.

What a Plateau Really Means

Daily scale readings are noisy. Sodium, bowel contents, menstrual changes, inflammation after exercise, and hydration can all mask fat loss. A true plateau is a stable trend over several weeks, not a few frustrating days.

As weight decreases, resting energy expenditure and the calories used during movement generally decline. Appetite biology may push back. The early water-weight component is also gone, so later progress looks slower.

Common Contributors

Dose escalation that is still in progress, missed doses, injection errors, liquid calories, alcohol, grazing, low protein intake, reduced activity, poor sleep, constipation, and medications that promote weight gain can all contribute. Thyroid disease, menopause, depression, sleep apnea, and other conditions may need review.

More medication is not always the answer. Going above an approved dose or accelerating titration can increase side effects without solving the underlying issue.

How to Respond Safely

Confirm adherence and the actual weight trend. Recheck calorie-dense extras rather than resorting to a crash diet. Prioritize adequate protein, resistance training, and daily movement to protect lean mass.

Use body-composition trends and waist measurements alongside weight. If the plateau persists, review diagnosis, dose tolerance, medications, sleep, and laboratory needs with the prescriber. Sometimes maintaining a clinically meaningful loss is itself a successful phase.
